The ds 82 only allows for a very narrow set of name changes, others you must use ds11.

USPS staff only take care of the shipping. The only form they will accept for processing is a DS-11 for a first-time passport application. This is also the only application form you'll be able to find in post offices. Renewal form DS-82 can only be downloaded directly from the State Department's website.

Note: You cannot download blank forms. After you have completed your form online, print it and sign inside the white box and date the application form. Make sure there are no alterations to signatures or dates. Three identical colour photos that are no more than six months old.
