The caption or title of a pleading is the heading or introductory clause which shows the name, address, telephone number of the attorney, the name of the parties, the name of the court, the county where the action is bought, and usually the term of the court and the number of the case on the docket or calendar.
422.30. (a) Every pleading shall contain a caption setting forth: (1) The name of the court and county in which the action is brought. (2) The title of the action. (b) In a limited civil case, the caption shall state that the case is a limited civil case, and the clerk shall classify the case ingly.
Rule 8.204 requires you to summarize the facts involved in the case “limited to matters in the record” (Rule 8.204(a)(2)(C), and also to support all of those matters “by a citation to the volume and page number of the record where the matter appears.” (Rule 8.204(a)(1)(C).)
