What to Include with the Complaint Form. A written Statement of Complaint that contains the information and facts on which the complaint is based (see details below). A list of Code of Ethics or Assistants Code of Conduct sections you believe, in good faith, that the Respondent violated.
A citizen may also file a sworn complaint by submitting a sworn affidavit alleging facts that support a specific violation of the Ethics Law. However, Commission rules require that the affiant demonstrate specific personal knowledge of all of the facts and evidence that support each element of a violation.
Include the heading/subject line: 'Formal complaint against name of lawyer or legal firm Identify who is making the complaint and the lawyer/firm you are complaining about.
