A signed Affidavit of Residency; and. 2. Your completion of this affidavit will facilitate the enrollment of your child(ren) (or of your own enrollment if you are an unaccompanied youth). 1. SECTION "B" ("AFFIDAVIT" STUDENT): Complete this section if the student is living with a person other than the parent or guardian. Doubled-up families do not need to submit this form.
