Wisconsin CLE Form 4 (Request for Approval of Continuing Legal Education Credit for Published Legal Writing) is a form provided by the State Bar of Wisconsin that enables attorneys to obtain continuing legal education (CLE) credit for legal writings that they have published. The form is used to document the CLE credit hours that a lawyer has earned for a published work, and it must be approved by the State Bar of Wisconsin before the credit can be awarded. There are two types of Wisconsin CLE Form 4 available: Type A (for articles/books/treatises) and Type B (for commentary/opinion pieces/blog posts). Both forms require the lawyer to provide information on the title, author, date of publication, publisher, target audience, length, and a description of the legal material covered. Additionally, the Type A form requires the lawyer to provide an abstract of the work, while the Type B form requires the lawyer to provide a copy of the work.
