Wisconsin Change of Name, Address, Employment Information is a form issued by the Wisconsin Department of Workforce Development to notify the department of changes to a person's name, address, and/or employment information. The form is used by individuals to update their records with the department, and may be required for certain benefits or programs. There are two types of Wisconsin Change of Name, Address, Employment Information forms: 1) Change of Name and Address; and 2) Change of Employment Information. The Change of Name and Address form is used by individuals to update their names and/or address with the department, and includes sections to report the individual's name, address, contact information, and any other changes. The Change of Employment Information form is used by individuals to update their employment information with the department, and includes sections to report the individual's employer, job title, full or part-time status, hours worked, and any other changes.
