Washington Application for Loss of Earning Power Compensation Medical (WALLOP) is a form used to apply for medical compensation if an individual has a disability that has resulted in a loss of earning capacity. The form is used by individuals in Washington State who are seeking medical compensation for their disability. WALLOP includes two types of applications: 1) the Initial Application and 2) the Reconsideration Application. The Initial Application is used to apply for medical compensation and includes information about the individual's disability, income, and medical expenses. The Reconsideration Application is used if the initial application is denied and includes additional information about the individual's disability and work history. The applications must be completed and submitted to the Washington State Department of Labor and Industries.
