The Washington Occupational Hearing Loss Questionnaire (WOULD) is a self-report survey designed to evaluate hearing loss associated with workplace exposure. It consists of 8 questions that assess the worker's subjective experience of hearing loss and the impact of this hearing loss on their daily activities. The WOULD is divided into four sub-scales: Hearing Loss Symptoms, Hearing Loss Severity, Hearing Loss Effects, and Hearing Loss Impact. The questionnaire can be used to monitor and track hearing loss over time and to identify workers who may need additional medical attention or referral. There are two versions of the WOULD: the Standard Version and the Short Version. The Standard Version contains all 8 questions, while the Short Version contains only 6 of the questions. Both versions are available in paper and online formats.
