The cover sheet includes a summary of the criminal history of the individual, including any arrests, charges, convictions, and related information. It also includes a Washington State Identification Number (WA SID) and a fingerprint card. There are two types of Washington Criminal History Record (Cover Sheet) (Non-Parent Custody): Court Ordered and Voluntary Disclosure. The Court Ordered Cover Sheet is used when the court has ordered a criminal history record for a non-parent seeking custody of a child. The Voluntary Disclosure Cover Sheet is used when the non-parent has voluntarily requested a criminal history record. Both types of cover sheets are used to ensure the individual's criminal history is properly documented.
