Washington Designation of Record (FOR) is an official document used by the Washington State Department of Health (DOH) to recognize the appointment of a guardian or conservator for an adult or minor who can no longer manage their own affairs. The Designation of Record documents the court-approved appointment of a guardian or conservator, and gives the individual the legal authority to manage the person's affairs. There are two main types of Does — Guardian of the Person and Guardian of the Estate. The Guardian of the Person is responsible for making decisions related to the person's care and well-being, while the Guardian of the Estate is responsible for managing the person's financial affairs. In addition, the FOR may also include a Health Care Directive or Power of Attorney for Health Care.
