Generally, a parent must file a Petition for Name Change on the child's behalf. The petition usually should be filed in the district court for the county where the minor lives. The petition is then sent to the judge for approval.

Fill out a Petition for Change of Name. It must state all of these: Your current legal name and the name you want. File a Petition in the district court of the county where you live. You must show photo ID when you file. Schedule a hearing date. The clerk will schedule a date for you to appear before a judge.

First, you file the Petition for Change of Name for your child. Then, you will get a court date between 6 and 12 weeks away. If the court approves your request, you will get a court order called a "decree" changing your child's name. The court process can take up to 3 months, though in busier courts it may take longer.

If one parent will not agree to have a child's name changed, the other parent can file papers to request the change. The non-consenting parent must be served with copies of the name change papers and given a chance to object. A judge may or may not grant a child's name change without the other parent's consent.

Contact your county's clerk of court and request the form to change the name of a minor (this is referred to as a petition). Complete the petition in full as missing information may result in your application being slowed or rejected. You will need to disclose family and educational history and more.

Effective July 1, 2019 the King County District Court fee for name changes is $201.50 which includes the $83 filing fee, a $10 administrative cost, a $103.50 recording cost per named individual and $5 for one certified copy. Additional certified copies are $5 per copy.
