Virginia Application for Adult Name Change (Adults Only) is a legal document used in Virginia for adults over the age of 18 to initiate a name change. This form must be filled out and notarized in order to legally change an individual's name in the Commonwealth of Virginia. The form must include the applicant's current name, their proposed new name, the reason for the name change, and other information, such as the applicant's birthdate, Social Security number, and address. In addition, two witnesses must sign the application, and the applicant must provide proof of publication of the proposed name change in a local newspaper. There are two types of Virginia Application for Adult Name Change (Adults Only): Petition for Change of Name, and Request for Change of Name. Both forms provide the same information as outlined above, but the Petition is used when the name change is being requested by the individual, and the Request is used when the name change is being requested by a legal representative.
