The Utah UCC 5 Information Statement is a one-page document that is used to record the filing of a UCC-1 financing statement with the Utah Department of Commerce. It is typically used by lenders to secure the repayment of a loan or debt, and generally includes the following information: the debtor's name and address, the secured party's name and address, description of the collateral being used as security, the date of filing, the file number, and the name of the county clerk or recorder where the UCC-1 was filed. There are two types of Utah UCC 5 Information Statements: a Blank UCC 5 Information Statement and a Verified UCC 5 Information Statement. The Blank UCC 5 Information Statement is used when a UCC-1 financing statement was filed without a signed verification, and the Verified UCC 5 Information Statement is used when a UCC-1 financing statement was filed with a signed verification.
