You must submit an application to add your domestic partner and any newly eligible children to your benefit plans and/or enroll in a benefit plan for the first time within 30 days of establishing a domestic partnership. All information below assumes you have established a domestic partnership.

You must apply in the County Clerk's office in the county where at least one of you has been living for the last 30 days; once filed, the declaration of domestic partnership will be valid anywhere in Wisconsin. If you must apply in another county, please contact the appropriate County Clerk's Office.

(1) Each individual is at least 18 years old and capable of consenting to the domestic partnership. (2) Neither individual is married to, or in a domestic partnership with, another individual. (3) The 2 individuals share a common residence.

Obtain the "Declaration of Domestic Partnership" form online or at the Clerk of the Circuit Court & Comptroller's main office in the Recording Department (Room 4.2500) or at any branch location. Complete the form so it is legible and have it notarized.

In legal terms, a domestic partnership is a committed relationship between two people who live together, but are not married. They share a residence, finances, and may raise children together as unmarried parents.

You are both mentally capable of entering into a contract. you are each other's only domestic partner. you aren't married to anyone else, and you aren't related by blood, and. if your relationship status changes, you will let the City Clerk know.

DOMESTIC PARTNER CRITERIA We are each other's sole domestic partner and intend to remain so indefinitely. We are not legally married to anyone. 2. We are at least eighteen (18) years of age, not related by blood closer than would bar marriage in the State of Iowa, and mentally competent to consent to this declaration.
