Texas Complaint for Review of Social Security Decision is a legal form used in the state of Texas to file a complaint in federal court so that the court can review a decision made by the Social Security Administration (SSA). This form can be used to request review of an SSA decision regarding Social Security benefits, Supplemental Security Income (SSI), Retirement, Survivors, or Disability Insurance (RSI), or Medicare. There are two types of Texas Complaint for Review of Social Security Decision: (1) A Complaint to Appeal a Denial of Benefits and (2) A Complaint to Appeal an Overpayment of Benefits. The Complaint to Appeal a Denial of Benefits is used when an individual has been denied benefits from the SSA and wishes to challenge the decision in federal court. The Complaint to Appeal an Overpayment of Benefits is used when an individual believes that they have been improperly overpaid by the SSA and wishes to challenge the decision in federal court. The Texas Complaint for Review of Social Security Decision must include the name of the plaintiff (the individual filing the complaint) and the defendant (the SSA), as well as a detailed description of the facts of the case and the grounds for review. The complaint should also include the name and address of the plaintiff’s attorney, if applicable.
