The Tennessee Return of Service form Circuit / Chancery is a form used in the state of Tennessee to document the service of process of a particular legal document. This form is used to inform the court that a legal document has been successfully served to the party in question, and serves as proof of service. The Tennessee Return of Service form Circuit / Chancery comes in two types: the Circuit Court Return of Service and the Chancery Court Return of Service. The Circuit Court Return of Service is used when serving papers for a Circuit Court case, while the Chancery Court Return of Service is used when serving papers for a Chancery Court case. Both forms will require the name and address of the person served, the date of service, the name of the party filing the papers, and a declaration of service. The form must also be signed and dated by the server.
