Uniform Civil Affidavit of Indigency

The Tennessee Uniform Civil Affidavit of Indigence is a form that is designed to allow individuals to request a waiver of court costs and fees associated with civil matters in Tennessee courts. This form is used to demonstrate to the court that the individual is unable to pay the costs and fees related to the case, and must be completed in full in order to be accepted by the court.
