It is usually signed by the defendant and the attorney and filed in the court where the case is being heard. The form includes the name of the defendant and the attorney, the attorney's bar number, the address of the attorney, and a statement of the attorney's agreement to represent the defendant. There are two types of Rhode Island Entry of Appearance — Criminal: an Entry of Appearance for Representation, which is filed when an attorney agrees to represent a defendant, and an Entry of Appearance for Appearance, which is filed when an attorney agrees to appear in court on behalf of the defendant.
