The Pennsylvania Notice to Retake Maiden Name (40.4 KiB) is a legal document used to allow a woman to take back her maiden name after marriage. This document is sometimes referred to as a “name change document” or a “name change affidavit.” The document states the legal name of the individual before and after marriage, as well as the county in which the marriage occurred. It also includes the signatures of both parties, witnesses, and a notary public. Pennsylvania Notice to Retake Maiden Name documents can come in two forms: a certified copy or an uncertified copy. A certified copy of the document must be signed by an authorized witness and a notary public, and is required to be submitted to the court when filing a petition for a name change. An uncertified copy of the document is accepted in some other instances, such as when applying for a driver's license or passport.
