The Pennsylvania Transcript Order Form is a document used by students and alumni of Pennsylvania schools to request an official transcript. This form can be used to order an official academic record of credits, grades, and other academic information. There are two types of Pennsylvania Transcript Order Forms, the Standard Form for requesting student transcripts and the Special Form for requesting non-student transcripts. The Standard Form requires information such as the student's name, address, Social Security Number, the school they attended, the year of graduation, and any other information requested by the school. The Special Form requires the same information, but may also require additional documentation such as a signed release form or an affidavit. Both forms require the payment of a processing fee. Upon completion and submission of the form, the transcript will be mailed to the requested address.
