Oregon Individual Debtors Certification Re Payment of Domestic Support Obligations (For Cases Filed On Or After 10-17-05) is a form used by individuals filing for bankruptcy in the state of Oregon. This form is used to certify that the debtor is making payments to fulfill domestic support obligations, such as child support or spousal support. The debtor must sign and date the form, and provide detailed information on the payment amount, date of payment, and who the payment was made to. There are two versions of this form: one for debtors filing under Chapter 7 bankruptcy and one for debtors filing under Chapter 13 bankruptcy. This form is required in order for the debtor's case to be approved and for the debtor to receive a discharge of their debts.
