The Oklahoma Juror Information Form — EFFECTIVE Nov. 1, 2013 is a form that is used by the court system in Oklahoma to collect pertinent information from prospective jurors. The form requests information such as the prospective juror's name, address, date of birth, driver's license number, Social Security number, telephone number, employer information, and other personal information. This form is also used to indicate whether the prospective juror has previously served on a jury, is a member of the military or law enforcement, or has been convicted of a felony. There are two types of Oklahoma Juror Information Form — EFFECTIVE Nov. 1, 2013: an online form for those who wish to submit their information electronically, and a paper form for those who prefer to submit their information by mail.
