Ohio First Report of Injury, Occupational Disease or Death is a mandatory report required by the Ohio Bureau of Workers' Compensation (BWC) for all workplace injuries, diseases or fatalities that occur in the state. This report is intended to provide injury and illness data to the BWC to help them improve safety and health in the workplace. It must be completed by employers within one business day after a work-related injury, disease or death has occurred.
