Case Information Sheet New York Case Information Sheet (CIS) is a document issued by the New York State Unified Court System that provides information about a case or proceeding, including the names of the parties involved, the case number, the court where the case is filed, the date the case was filed, the judge assigned to the case, and the status of the case. CIS is generally used by attorneys, legal researchers, and other court personnel to track cases in the state court system. There are two types of New York Case Information Sheet: Civil Information Sheet (CIV-CIS) and Criminal Information Sheet (CRM-CIS). CIV-CIS is used for civil cases and provides information such as the name of the court where the case is filed, the case number, the names of the parties involved, the date the case was filed, the judge assigned to the case, and the status of the case. CRM-CIS is used for criminal cases and provides similar information as the CIV-CIS but also includes the charge, the date of the charge, the arresting officer, and the arresting agency.
