The New York Freedom of Information Appeal Form is a document used to request access to records that have been denied by a state or local government agency. It enables individuals to make an administrative appeal of an agency's denial of their request for access to records. The form can be used to challenge a government agency’s refusal to provide records or to challenge the amount of information that an agency has provided. There are two types of New York Freedom of Information Appeal Form: the general appeal form and the appeal for records that involve an individual’s own personal information. The general appeal form is used for all other requests for records. The appeal for records that involve an individual's own personal information is used when an individual requests records that contain his or her own personal information. Both forms require an explanation of why the individual believes that the agency's decision to deny access was wrong.
