The Nevada Notice of Intent to Appear Telephonically Nonrefillablele Form (known as Form CJA-4) is a legal document used by the Nevada court system. It is used by a party to notify the court that they intend to appear telephonically at a hearing. This form is not fillable and must be printed and signed by the party. It includes the name of the court, the case number, the names of the parties, the date and time of the hearing, and the telephone number the party will use to participate in the hearing. There are two types of Nevada Notice of Intent to Appear Telephonically Nonrefillablele Form; one for criminal cases and one for civil cases.
