This form is available as an online form on the Nevada DMV website or can be obtained in person at a local DMV office. There are two types of Nevada Notice of Change of Address: one for individuals and one for businesses. The individual form requires the applicant's name, date of birth, driver's license number, and social security number. The business form requires the business name, address, and federal employer identification number. Both forms also require the new address and the applicant's signature. Once the form is completed, it must be returned to the Nevada DMV either in person or by mail.
