New Jersey IFSA — Certification in Support of Non-Disclosure (for use with Personal Information Form for IFSA 311) is an official document created by the New Jersey Department of Human Services to be used with the Personal Information Form for IFSA 311. This document allows an individual or their representative to certify that certain information provided in the Personal Information Form for IFSA 311 should not be disclosed to the public. This form must be filled out and signed by the individual or their representative, and then submitted to the New Jersey Department of Human Services. There are two types of New Jersey IFSA — Certification in Support of Non-Disclosure: Type I, which is for individuals, and Type II, which is for representatives. Both types of certification must be completed in order to obtain an IFSA 311 form.
