The New Jersey Adult Guardianship — Case Information Statement (CIS) is a form used by the court to provide information about a case for guardianship of an adult. This CIS is used in the state of New Jersey to provide information to the court regarding the proposed guardian and the proposed ward (adult needing a guardian). This form includes information about the proposed guardian, the proposed ward, the need for guardianship, assets and income of the ward, and other relevant information. There are two types of New Jersey Adult Guardianship — Case Information Statement (CIS): the initial CIS and the periodic CIS. The initial CIS is used for the initial filing of a guardianship case, while the periodic CIS is used for filing a renewal or modification of the guardianship.
