Family Additional Information Sheet

The New Jersey Family Additional Information Sheet is a document used by New Jersey State agencies to obtain additional information needed to process applications for various public assistance programs. The document is used to collect information from applicants including household composition, income, expenses, and other pertinent information. The information provided on the sheet is used to determine eligibility for public assistance programs, such as the Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANK), and the Low Income Home Energy Assistance Program (LIH EAP). The New Jersey Family Additional Information Sheet comes in two versions: the Household Information Sheet (HIS) and the Income and Expense Sheet (IES). The is used to collect information about the composition of the applicant's household, such as the number of persons living in the household, their relationship to the applicant, and their ages. The IES is used to collect information about the applicant's income and expenses, such as their wages, Social Security benefits, and other sources of income, as well as their expenses, such as rent, utilities, and other bills.
