The New Jersey Civil Cover Sheet (JS44) is a form used to initiate a civil action in New Jersey state courts. It is used by the plaintiff or petitioner to provide the court with basic information about the case. The cover sheet must be filed with the complaint and includes information such as the titles of the parties, the nature of the action, the jurisdiction of the court, and the relief being sought. It is important to note that the Civil Cover Sheet is not a substitute for the complaint, and all information provided must be accurate. There are two types of New Jersey Civil Cover Sheet (JS44): the JS44A for a Complaint or Notice of Appeal, and the JS44B for an Answer, Counterclaim or Cross-Claim. Both forms are available on the New Jersey Judiciary website.
