The New Hampshire Report of Extended Disability (SHRED) is a form that an individual completes to apply for Social Security Disability benefits. It is used to determine whether an individual meets the criteria for Social Security Disability and to provide the Social Security Administration (SSA) with information about the applicant’s medical condition and work history. The form consists of two sections: the Medical Report and the Work History Report. In the Medical Report section, the individual must provide detailed information about their medical diagnosis, treatment, and prognosis. This includes information about diagnoses, any current and past medical treatments, the individual’s functional capacity, and any other related information. The Work History Report section asks the applicant to provide detailed information about their employment history, including job titles, job duties, dates of employment, and any other relevant information. There are two types of SHRED reports: the initial report and the reconsideration report. The initial report is the first step in the disability application process and is used to determine whether an applicant meets the initial qualifications for Social Security Disability. The reconsideration report is used in cases where an applicant has been denied Social Security Disability and is requesting a reconsideration of their case.
