The Nebraska Address Update Affidavit is a form used by the Nebraska Department of Motor Vehicles (DMV) to update an individual’s address on their DMV records. This form is necessary for driver’s license holders and vehicle owners in the state of Nebraska when they move to a new address within the state. It is also used to update the address associated with existing Nebraska vehicle registrations. There are two versions of the Nebraska Address Update Affidavit form: one for a change of address within the same county, and one for a change of address to a different county. Both forms must be signed in front of a notary public and include the individual's current address, new address, driver’s license number, and vehicle registration number (if applicable). The DMV will then update their records with the new address.
