North Carolina Appearance of Child Support Creditor or Representative is a form used by a creditor or representative of a creditor to appear in a North Carolina court to present evidence that a debtor has not paid child support as ordered by the court. This form is required for a creditor or representative to appear in court and is used to provide the court with information about the creditor's or representative's identity, contact information, and any evidence they may have regarding the debtor’s non-payment of child support. There are two types of North Carolina Appearance of Child Support Creditor or Representative forms: one for an individual creditor or representative and one for an organization or corporate representative. Both forms require the creditor or representative to provide details such as name, address, phone number, and email address, as well as a brief description of the evidence they have regarding the debtor’s non-payment of child support.
