The DOW is used to assess and document the patient's vision, eye health, and overall ophthalmologic status. The DOW includes various sections, such as a visual acuity test, general ophthalmic examination, funds examination, and pupillary assessment. Each section includes questions and prompts to help the physician document the patient's ophthalmologic condition. There are two types of DOW forms: the Standard DOW and the Specialized DOW. The Standard DOW is used for general ophthalmologic evaluations, while the Specialized DOW is used for more specific evaluations, such as those for glaucoma, diabetes, or retinal detachment.
