Michigan Income Withholding Information is a requirement for Michigan employers to collect income tax from employees’ wages and remit it to the Michigan Department of Treasury. There are two types of Michigan Income Withholding Information: Form 4134, which is used to report income withholding from wages and salaries; and Form 4164, which is used to report income withholding from other sources such as pensions, annuities, and sales commissions. Both forms include information such as employee name, Social Security Number, employer name, employer address, and total income subject to withholding. Employers must submit these forms on a quarterly basis in order to remain compliant with Michigan income tax laws.
