Funeral Establishment License, Relicensure And ChangeOfName And/Or Manager Application The Michigan Funeral Establishment License, Relic ensure And ChangeOfName And/Or Manager Application is a set of forms used to apply for or renew a license for a funeral establishment in the State of Michigan. It is required for any business that provides funeral services, including embalming, arranging, and conducting funerals, and the sale of caskets, urns, and other funeral products. The application includes three types of forms, each of which is used for a different purpose: 1. Funeral Establishment License Application: This form is used by any new business that wants to apply for a funeral establishment license in Michigan. 2. Funeral Establishment Relic ensure Application: This form is used to renew an existing license for an existing funeral establishment. 3. Funeral Establishment Change of Name and/or Manager Application: This form is used to change the name of an existing funeral establishment or to appoint a new manager of the business.
