The Maine Civil Cover Sheet is a form filed with the court that provides the court with basic information about the parties involved in a civil lawsuit. It is typically the first document filed with the court and is used to initiate a civil action. The Maine Civil Cover Sheet includes the names and addresses of the parties, the court in which the action is brought, the case number, the title of the case, a statement of the jurisdiction of the court, a statement of the damages sought, an optional statement of the nature of the cause of action, and the names of any attorneys or law firms representing either party. There are two types of Maine Civil Cover Sheet: one for Complaints and one for Counterclaims.
