Louisiana Title IV, Appendix 30.0A. Sworn Detailed Descriptive List is a form used by the Louisiana Department of Revenue to provide a detailed description of taxable property in the state. This form is used to report the items listed in the form and any other items that the taxpayer wishes to report. The report must include a description of the item, its value, its location, and any other information specified in the form. The form is typically used for personal property (such as furniture, vehicles, jewelry, and antiques) or business assets (such as equipment and machinery). There are two types of Louisiana Title IV, Appendix 30.0A. Sworn Detailed Descriptive List: the Individual Form and the Business Form. The Individual Form is used to report personal property owned by an individual taxpayer, while the Business Form is used to report business assets.
