The Louisiana Doctor Choice Form is an online form that allows individuals to select a primary care physician (PCP) and other specialists from the Louisiana Health Care Connections network. The form can be accessed through the Louisiana Health Care Connections website or app and is available to those who have health coverage through Louisiana Medicaid, Lac HIP, or a Qualified Health Plan. There are two types of Louisiana Doctor Choice Forms: one for individuals and one for families.
