Louisiana Affidavit (Null and Void) of Inscriptions on Certificates is a document issued by the state of Louisiana in accordance with La. R.S. 9:2743. This document is used to nullify and void any inscriptions made on a certificate, such as a birth, death, marriage, or divorce certificate. The Louisiana Affidavit (Null and Void) of Inscriptions on Certificates is signed by an authorized notary public, and includes the name of the person whose certificate is being nullified, as well as a description of the inscription being voided.
