A Louisiana Notice of Lawsuit & Request for Waiver of Service of Summons is a document used to notify a defendant that a lawsuit has been filed against them. It provides the defendant with the information needed to respond to the complaint, including the name of the court in which the case is being heard, the case number, the name of the plaintiff and defendant, and the date the complaint was filed. The Notice of Lawsuit also includes a Request for Waiver of Service of Summons, which requests that the defendant waive their right to be served with a physical summons, and instead acknowledge receipt of the Notice by signing and returning it. There are two types of Louisiana Notice of Lawsuit & Request for Waiver of Service of Summons: the Original Process Notice and the Summons by Publication Notice.
