The Dissociation of a Sole Member is used when two parties are dissociating and one is the sole member of the association. The Dissociation of Joint Members is used when two parties are dissociating and both are joint members of the association. The Kentucky Statement of Dissociation must be signed by both parties in the presence of a notary. It must also include the date of the document, the names of all parties involved, and the specific reason for the dissociation. Upon completion, the document should be filed in the county where the association was formed.
