The document must include the business entity’s name, its type of entity, its current address, its new address, the mailing address of the new address, and the effective date of the change. The document must also be signed and dated by an authorized representative of the business entity. Once filed, the Kentucky Secretary of State will update the business entity’s records to reflect the new address.
