The Summary Form includes additional information such as the date of the claim, the name of the creditor’s attorney, and the name of the creditor’s surety, if any. The Detailed Form includes all the information of the Short Form as well as the type of debt, the date of the claim, and the date of the claim’s filing. All three forms must be filed with the court before the court can enter an order of discharge.
