Illinois Consent To Exercise of Jurisdiction By US Magistrate Judge is a form that allows a US Magistrate Judge to exercise jurisdiction in a civil case in the US District Court for the Northern District of Illinois. This form is used when the parties agree to have a US Magistrate Judge preside over a case, rather than have it heard by a US District Judge. There are two types of Illinois Consent to Exercise of Jurisdiction By US Magistrate Judge forms: one for individual litigants and one for corporate litigants. The individual litigant form requires the signature of the litigant and his/her attorney while the corporate litigant form requires a corporate representative signature. Both forms must be signed in the presence of a notary public or other qualified witness. The form should then be filed with the Clerk of Court along with the other necessary documents for the case.
