The Illinois Report of Attorney-in-Fact or Consul is a legal document filed with the Illinois Secretary of State in order to authorize a business or individual to act on behalf of another person or entity. The document is commonly used in the formation of a corporation, limited liability company, partnership, or non-profit organization. There are two types of Illinois Report of Attorney-in-Fact or Consul: a Domestic Report and a Foreign Report. A Domestic Report is used when the attorney-in-fact or consul is domiciled in Illinois, while a Foreign Report is used when the attorney-in-fact or consul is not domiciled in Illinois. Both reports must include the name and address of the attorney-in-fact or consul, the name of the appointing person or entity, the purpose of the appointment, and the name of the business or organization in which the attorney-in-fact or consul is to act. The Secretary of State must be notified of any changes to the report within ten days of the change.
