An Illinois Notice of Appeal is a legal document that is filed with the Illinois Appellate Court when a party wishes to appeal a decision made in a lower court. This document is used to inform the court that an appeal is being made and to provide the court with relevant information about the case. The Notice of Appeal must include the names of the parties involved in the case, the names of the lower court and the Appellate Court, the date of the lower court’s decision, the grounds for the appeal, and the relief being sought. There are two types of Illinois Notice of Appeal: a Notice of Appeal from a Circuit Court and a Notice of Appeal from an Administrative Agency. Each type of Notice of Appeal must be completed and filed with the Appellate Court in the same manner.
