The Hawaii Certificate of Service is a document issued by the Hawaii State Department of Health that serves as proof that a person has served in the U.S. Armed Forces, National Guard, or other military service in Hawaii. It is commonly used for verifying service-connected disability benefits, veteran’s benefits, and other military-related benefits. There are two types of Hawaii Certificate of Service: the Hawaii State Certificate of Service and the National Guard Certificate of Service. The Hawaii State Certificate of Service is issued to those who served in any branch of the U.S. Armed Forces; the National Guard Certificate of Service is issued to those who served in the Hawaii National Guard. Both certificates are issued in the form of a plastic card with a unique identification number.
