The Hawaii Application for Reinstatement is a document used to reinstate a person’s Hawaii driver’s license or instruction permit after it has been revoked or suspended. The Hawaii Department of Transportation (HOT) requires applicants to submit a completed Application for Reinstatement form, along with a $25 fee and any other applicable forms and documents, to be considered for reinstatement.
